What is Preservation Rhinoplasty?

To understand preservation rhinoplasty, imagine your nose is a house with a steep roof (the nasal bridge) and you want to lower it. 

In a traditional renovation (nose surgery), you’d tear the roof off, cut the walls down, and build a new roof. This breaks the natural 'keystone area of the nose,' the critical junction where bone meets cartilage. 

While effective, it disrupts the smooth, natural lines of the nasal bridge.

In preservation rhinoplasty, we keep the roof intact. Instead of cutting the top off, we remove a small strip of foundation from the bottom (deep inside the nose) and gently 'push down' the entire structure. 

The result? The roof becomes lower, but the smooth, natural ridge of your nose remains completely untouched. We avoid the irregularities, bumps, and 'inverted V' deformities that can sometimes occur when the bridge is cut open and reconstructed.

This modern rhinoplasty method allows us to reduce a dorsal hump or refine a profile while keeping the essential ligaments, skin, and valves intact. This is why preservation rhinoplasty surgery creates a more natural look.


Preservation vs. Traditional Rhinoplasty

The difference lies in what we take away versus what we keep. 


Traditional (Structure) Rhinoplasty

This is 'reductive.' To remove a bump, the surgeon cuts off the top of the nasal bridge. This breaks the natural connection between the cartilage and bone. The surgeon then has to rebuild the nose using grafts to restore, strength, and smooth it out.

Traditional rhinoplasty can sometimes result in contour irregularities or a 'pinched' look if healing is erratic.


Preservation Rhinoplasty

This is 'conservative.' Instead of cutting the top, we remove small strips of cartilage and bone from underneath the bridge. This allows the bridge to drop naturally to the desired height, and the structural integrity of the nose remains 100% original.

Preservation rhinoplasty creates a more natural and balanced look because the native anatomy isn't disrupted, only repositioned.

What to Expect During Recovery

  • Week 1: You'll wear a splint to protect the new shape. Congestion is normal.

  • Week 2: The splint comes off. Most bruising (if any) has faded. You can return to work or school.

  • Months 1-3: Your nose settles. Because the 'roof' was never separated, the bridge heals very reliably.


Frequently Asked Questions About Preservation Rhinoplasty 

Can this fix a bulbous tip? 

Preservation techniques focus mainly on the bridge (dorsum). However, we almost always combine it with tip refinement techniques to ensure the tip balances perfectly with the new bridge height.


Is the recovery really faster? 

Yes, for most patients. Because we preserve the natural layer right above the cartilage, we don't disturb the blood vessels and lymphatics that cause massive swelling. You look 'normal' much sooner.


What if I have a deviated septum? 

We can absolutely correct a deviated septum during the same surgery. In fact, septal correction is a core part of the 'foundation work' in preservation techniques.


Can this be done for revision surgery? 

Usually, no. Preservation relies on the original ligaments and structures being intact. If a previous surgeon has already cut the bridge, we typically must use structural grafting techniques to rebuild it.


Does the hump grow back? 

No. Once the bone and cartilage are repositioned and healed in their new location, the change is permanent. The 'push down' technique creates a stable new foundation for the nose.


Is this 'open' or 'closed' surgery? 

Preservation rhinoplasty can be performed via both approaches. However, we often use a 'closed' (scarless) approach or a very minimal open approach, depending on whether we need to refine the nasal tip simultaneously.


Does it help with breathing? 

Yes. Better breathing is a benefit of preservation rhinoplasty. Because we don't narrow the internal nasal valve (the narrowest part of the airway) as much as traditional reduction might, airflow is preserved or improved.


Is the recovery painful? 

Most patients describe the recovery as 'uncomfortable' rather than painful. You'll feel congestion, similar to a heavy cold. Because preservation techniques cause less trauma, many patients report using less pain medication than expected.


How long until I see results? 

You'll see the change immediately upon cast removal (usually day 7). However, subtle swelling will persist for months. The final definition of the tip generally settles around the one-year mark.
