A scar is a story, but not every story needs to be visible on your face. Whether caused by trauma, acne, or a past surgery, facial scars can impact how the world sees you, but, more importantly, how you see yourself.

Understanding the Canvas: How Scars Form


Before we can treat a scar, we must understand its nature. Scars form when the deep, thick layer of skin is damaged. The body rushes to repair the wound by producing collagen fibers. 

Unlike normal skin, where these fibers are woven like a basket, scar tissue fibers align in a single direction, creating a different texture and color.


Types of Scars

  • Atrophic Scars. Depressed or sunken (common with acne).

  • Hypertrophic Scars. Raised and red, but contained within the wound boundary.

  • Contracture Scars. Tight skin that can restrict movement.

Non-Surgical Options: Polishing the Surface


Not all scars require a scalpel. For scars that affect surface texture or color, we often start with non-invasive methods. These are excellent for smoothing irregularities and blending the scar with surrounding skin.


Laser Scar Removal

We use advanced laser technology to resurface the skin, effectively smoothing out the 'hills and valleys' of a scar.

PRP Microneedling Scars

We also use PRP microneedling scar treatments for depressed or acne scars. 

We create thousands of microscopic channels in the scar tissue using fine needles. This physical disruption breaks up old, stiff collagen bundles. We then apply Platelet-Rich Plasma (PRP), derived from your own blood, flooding the area with growth factors. 

PRP microneedling effectively 'reboots' the healing process, encouraging the skin to produce softer, more organized collagen. This biological boost encourages the skin to 'fill in' depressions and normalize texture naturally.


Surgical Scar Revision: Structural Restoration


Sometimes, a scar is too deep, wide, or misaligned with the natural tension lines of your face for lasers alone. 

In these cases, surgical scar revision is necessary to physically alter the scar’s structure. This involves removing the old scar tissue entirely and re-closing the wound with precise techniques.


Breaking Geometric Lines 

Our eyes are trained to notice straight lines because they rarely occur in nature. This is why a long, straight scar is so easy to spot. 

To counter this, Dr. Mo Boonipat uses techniques like W-plasty or Z-plasty. These methods involve surgically breaking the scar into a zigzag or irregular pattern. By redirecting the scar into the face’s natural resting lines (wrinkles or contours), we 'trick' the eye into missing the scar entirely. 

It's an optical illusion created through surgery.


Fat Grafting

For depressed or 'scooped out' scars or scars 'tethered' down to the muscle or bone, we need to restore volume. To do this, we can take fat from another part of your body and inject it under the scar. 

Fat grafting lifts the depression or tethering to the same level as the surrounding skin, eliminating the shadow that makes the scar visible.


The Multi-Modality Approach for Facial Scar Revision


Unfortunately, there are no 'wonder cures' for scars, especially complex scars. The best results often come from a combination of therapies, what we call the 'FacePLUS+ Protocol.'

For example, we might perform surgical scar revision to narrow a wide scar, followed by laser treatment six weeks later to smooth the edges, and finish with microneedling to perfect the texture. 

This layered approach ensures that we attack the scar from every angle—depth, width, color, and texture.


Recovery and Aftercare


Patience is your most important tool during recovery. Facial scar revision is a process of maturation, but it’s typically milder than the original injury.

So, what should you do? 

  • Sun Protection. This is non-negotiable. New skin is highly sensitive. UV rays cause fresh scars to darken (hyperpigmentation). You must use SPF 30+ and keep the area shaded for at least 6 months.

  • Silicone Therapy. We often prescribe silicone gel or sheets to hydrate the scar and regulate collagen production. This helps keep the scar flat and pale as it matures.

  • Patience. While you'll see immediate changes, the final 'invisible' result can take 6 to 12 months as the tissue settles.


Frequently Asked Questions About Facial Scar Revision


Can a scar be completely removed? 

Medically speaking, no. Once the skin is cut, a scar will always exist. However, our goal is revision and functional invisibility. This means making the scar look like a natural skin crease so that it’s no longer so noticeable. 


Am I a candidate for laser scar removal in Lubbock?

Most patients with surface-level irregularities or discoloration are good candidates. However, patients with active infections or certain skin types may need specific preparation.

Does scar revision surgery hurt? 

The procedure is performed under local anesthesia or sedation. You'll feel no pain during the procedure, and post-op discomfort is usually mild and easily managed with over-the-counter medication.


When is the best time to revise a scar? 

We typically wait at least 6 to 12 months after the initial injury to allow the scar to mature. However, in cases where a scar is distorting a feature (like pulling on an eyelid or lip), we may intervene sooner.


Will my insurance cover this? 

If the scar restricts movement (like near the eye or mouth) or is caused by trauma or cancer, insurance may cover it. Purely cosmetic revisions are typically out-of-pocket. We can help verify your benefits.
